We would like to inform you that we are closing our practice in Allentown on Wednesday, May 31, 2022. We are moving to Canada to be close to our family.

During our residency program in Allentown, and in the last year at our private practice, we have come to know many people as friends and patients. You have been very kind to us and we appreciate that very much. We will be carrying with us the wonderful memories of Allentown.
Your medical records may be released to another provider with a signed consent form (mailed). Please complete it and text (484-820-1234) or email (hello@majeedhealth.com) us a copy so we can send your records to your next doctor. To get a list of doctors in your insurance plan, call the number on the back of your insurance card. You can visit our website (www.majeedhealth.com) for updates.
